Spectrum Health Continuing Care (SHCC) has two openings for experienced RNs.
Access Management Coordinator
Are you a nurse who is a stay at home mom during the week who would like a position that balances career and family needs?
The SHCC Access Management Department is looking for an RN with at least 5 years of clinical experience in home health/hospice and long-term care; is knowledgeable about the post-acute continuum of care and reimbersement; and is an excellent team player and communicator. This position will work every other weekend 8 hours on Saturday and 4-5 hours on Sunday to faclitate home health and hospice referrals to SHCC program from hospitals.
Access Management Coordinator
Full time position available. The Access Coordinator manages referrals to SHCC programs and facilitates a smooth transition of patients into these programs. Maintains communication with referral sites, external case managers, and insurances companies. RN with 5 yeras experience in a rehabilitation or post-acute care setting. Prior experience as case manager desired. Demonstrated excellent customer service skills a must.
